• New referral pathway

  We have made some changes to the way schools request access to interventions funded by SAP and allocated through the SAP Panel. Please send all requests to our new email address 

  referralssap@churchvale.notts.sch.uk. We also have a new referral form which replaces the paperwork for each individual intervention. There is now one referral form for all interventions and a section for schools to select the intervention being requested. You can find the new form on this website by visiting the counselling page or Dukeries equine sessions page. 

  We hope these changes will simplify the system for schools and the SAP team.
